Metabolic Challenges

Obesity, diabetes, and metabolic syndrome (“Syndrome X” – encompassing insulin resistance, type 2 diabetes, dyslipidemia, hypertension, and abdominal obesity) are reaching epidemic proportions in the United States and around the world. Excess glucocorticoids such as cortisol (a primary stress hormone) have been theorized to contribute to visceral (abdominal) obesity and diabetes.  Our body undergoes major changes past 30 years of age.  Science has shown a clear link between metabolic syndrome and adipose tissue.  While the formation of adipose tissue results for a variety of reasons, in many adults this occurs due to changes in hormone balance.   Specifically, there is a decrease in Androgen hormones (DHEA, Testosterone, etc.) and an increase in Sex Hormone Binding Globulin.  While these types of hormones are primarily associated with sexuality, they act in concert with other hormones and control a wide array of metabolic functions. 

 

Its import to understand that adipose formation is not bad because adipose tissue is actually a part of the endocrine system, creating a number of hormones to help the body support endocrine deficiencies.  However, it can also over form and cause a host of health imbalances.

 

Anabolic vs Catabolic State

One way of categorizing metabolic processes, whether at the cellular, organ or organism level is as anabolic or catabolic. Catabolism is the part of metabolism that breaks down molecules into smaller units to generate energy and simultaneously takes measures to conserve energy. Stress, such as during weight loss program, can put the body into the catabolic state, in which the body can experience muscle loss, reduced metabolic rate with a corresponding reduced calorie expenditure and lower perceived energy levels, and weight gain in the form of fat. One aspect of the catabolic state is that it can be characterized as having high cortisol (a catabolic stress hormone) levels and low testosterone (an anabolic hormone) levels.

 

Conversely, in the anabolic state the body experiences muscle maintenance or growth, normal metabolic rates, and weight loss in the form of fat. The anabolic state can be characterized by relatively low cortisol levels and high testosterone levels. 

 

11(beta)-HSD1 enzyme:  11(beta)-HSD1 is an enzyme that plays an important role in the cellular interconversion of glucorticoids between the active (cortisol) and inactive (cortisone) forms. Thus, 11(beta)-HSD1 is responsible for the modulation of cellular cortisol exposure and as such plays an important role in a number of health imbalances associated with excess glucocorticoid.  11(beta)-HSD1 is present in the liver and the adipose tissue. 11(beta)-HSD1 amplifies local glucocorticoid concentrations in these target tissues, even when circulating cortisol levels are low. In obese subjects, adipose levels of 11(beta)-HSD1 levels are markedly increased and animal models of elevated 11(beta)-HSD1 activity demonstrate visceral obesity, insulin resistance, diabetes, and dyslipidemia. Inhibition of 11(beta)-HSD1 in animals (pharmacologic and genetic) and humans (pharmacologic) results in enhanced insulin sensitivity.

 

The 11(beta)-HSD1 enzyme modulates tissue-specific glucocorticoid concentrations by generating active cortisol. Previous research has shown both liver 11(beta)-HSD1 and adipose tissue 11(beta)-HSD1 to be associated with hyperinsulinemia (diabetes) and visceral adiposity (abdominal obesity), respectively.   Rodent models of diabetes and obesity display elevated 11(beta)-HSD1 expression and transgenic mice with elevated 11(beta)-HSD1 develop visceral obesity, diabetes, and elevated cholesterol levels. Transgenic “knockout” mice (which lack 11(beta)-HSD1) are resistant to diabetes and obesity, even when fed a high-calorie diet. Obese and diabetic human subjects have been shown to exhibit elevated 11(beta)-HSD1 levels and activity – and inhibition of 11(beta)-HSD1 activity with selective or non-selective agents has been shown to improve insulin sensitivity.

Citrus PMFs: Citrus peel extract includes Polymethoxylated Flavones (PMFs), particularly tangeretin, sinensetin, and nobilitin, which are known to have health benefits in maintaining healthy cholesterol level and promoting cardiovascular health. PMFs represent a class of “super flavonoids” extracted from citrus peels that exhibit approximately 3-fold potency in their ability to reduce cholesterol levels as compared to other citrus flavonoids.  Citrus PMF inhibits the 11(beta)-HSD1 enzymes, thereby reducing systemic and local cortisol concentrations (liver and adipose tissue), promoting healthy blood sugar control and healthy weight loss.

 

Eurycoma Longifolia:  Eurycoma (Eurycoma Longifolia, also known as Tongkat ali and Malaysian ginseng) specifically the Eurypeptides, have known to increase energy levels and enhance libido in older men.  Eurypeptides are long chains of amino acids helps enhance weight loss and maintain high energy levels by maintaining normal levels of cortisol and testosterone during weight loss (anabolic state). More particularly, Eurycoma is used to help maintain normal (low) cortisol and normal (high) testosterone levels during the stress of weight loss. This hormonal control provides energy to a person in a weight loss phase while simultaneously helping them lose weight. As a result, effective doses of Eurycoma help prevent the body from seeking to gain weight by storing fat and increasing appetite, thereby help stop the “yo-yo” diet effect where a dieter’s initial weight loss of a few pounds sends the body into a catabolic state, leading to binge eating and fat storage. In addition, it is believed that inhibiting or reducing the cortisol spike associated with weight loss can make it easier for a person to control their appetite. Similarly, it is believed that controlling the cortisol and testosterone levels helps prevent the body from increasing fat storage and increases a person’s baseline energy.
